Research Progress and Mechanisms in the Diagnosis and Treatment of Severe Acute Pancreatitis

Abstract

Acute pancreatitis (AP) is one of the most frequent and potentially fatal gastrointestinal emergencies in the world, and its severe form (SAP) has been associated with high mortality rates of 20%-40%. Although there have been great strides in knowledge of pathophysiological processes and treatment technologies in the last ten years, SAP continues to be a significant clinical problem globally. This review critically reviews recent advances in the understanding of AP pathogenesis, biomarkers of diagnosis, imaging assessment, therapeutic approaches, and management of complications, focusing especially on acinar cell damage and abnormal proenzyme activation, inflammatory cascade responses and cytokine storm, microcirculatory disturbance and intestinal barrier dysfunction, and pancreatic necrosis infection as central pathological mechanisms. The article highlights the core principle of personalized therapy, multidisciplinary team (MDT) cooperation, and minimally invasive interventional technology in enhancing patient care, and provides insight into the future research directions such as the creation of highly specific early warning biomarkers, optimization of step-up approach technical specifications, validation of clinical translation value of new therapeutic targets including inflammatory modulation and stem cell therapy, and development of big data-based prognostic prediction models, to give an exhaustive evidence-based guidance in clinical decision-making.
