Comparison of the Effects of Knee Unicondylar Replacement Versus Open Wedge High Tibial Osteotomy Treatment on Knee Joint Motor Function and Mobility in Patients with Grade K-LIII Medial Compartment Knee Osteoarthritis

Abstract
Objective: Compare and analyze the specific effects of knee unicondylar replacement and Open Wedge High Tibial Osteotomy (OWHTO) treatment on the knee joint motor function and athletic ability of patients with K-LIII grade knee medial compartment osteoarthritis. Methods: 100 cases of K-LIII grade medial compartmental knee osteoarthritis patients seen in the Department of Orthopaedics of the hospital from December 2022 to December 2023 were selected and randomly grouped according to the lottery method, with 50 cases in each group. 50 cases were treated with Unicompartmental Knee Arthroplasty (UKA), which was set up as the UKA group, and 50 cases were treated with OWHTO, which was set up as the UKA group. The other 50 cases adopted the OWHTO, which was set as the OWHTO group; the knee joint function, knee joint mobility, and postoperative complications of the two groups were compared. Results: All the knee motion function indexes of the two groups at 6 months after the operation were significantly improved compared with those before the operation, and the knee flexion angle value of the patients in the UKA group was higher than that of the OWHTO group, and its extension angle value and stabilization value were lower than that of the OWHTO group, P < 0.05. The 6-min walking distance and Tegner knee motion score of both groups at 6 months after surgery were significantly improved compared with the preoperative period, and the 6-min walking distance of patients in the UKA group was longer than that of the OWHTO group, and their Tegner knee motion scores were higher than those of the OWHTO group, P < 0.05. The postoperative complication rate was 8.00% in the UKA group and 10.00% in the OWHTO group, and there was no significant difference in the comparison between the groups, P < 0.05. Conclusion: UKA is more effective than OWHTO in treating patients with osteoarthritis of the medial intercalary compartment of the knee of grade K-LIII, and it is especially suitable for patients who are older, with a low demand for activity, and whose lesions are mainly concentrated in a single compartment.
